Adult Social Care Online

Description

This is a new Adult Social Care Online service from Surrey County Council which lets local Surrey residents complete online forms quickly and easily, 24 hours a day, seven days a week and at a time that suits them. These online forms will:

  • Tell people if they are likely to qualify for social care support 
  • Signpost people to local services that are relevant to their needs
  • Help people to decide whether to continue with a more detailed assessment
  • Tell people if they are likely to have to pay for support, and estimate how much that may be.

There are five options in total:

  • An Adult Social Care Support Needs Checklist which gives people an early indication of likelihood to receive social care support;
  • An Adult Social Care Support Needs Assessment that is a full assessment of an individual’s social care support needs, completed online;
  • An Adult Social Care Carers’ Support Needs Checklist which gives people an early indication of likelihood to receive social care support as a carer;
  • An Adult Social Care Carers’ Support Needs Assessment that is a full assessment of a carers support needs, completed online;
  • An Adult Social Care Online Financial Assessment to establish how much a person may have to pay towards their care. The person will need to have certain key documents to hand before completing the form.
